Terms & Conditions

1. Services Provided

ClaimRev provides medical claims clearinghouse and revenue cycle management services. Services include claim review, electronic payer submissions, and eligibility verifications.

2. Client Responsibilities

• Ensure claims are monitored and corrected in a timely manner
• Keep account information accurate and up to date
• Protect system login credentials

3. Termination & Data Access

Clients may request termination in writing. Access to the account will remain available for 60 days at a discounted rate for data transfer. After that, data will be purged according to HIPAA-compliant retention policies.

4. Limitation of Liability

ClaimRev is not liable for any unpaid or delayed claims or discrepancies due to client inaction or payer-side changes. Reimbursement is not guaranteed. Insurance coverage updates post-verification are beyond our control.

6. Intellectual Property

All website content, including branding, software, and service documentation, is the intellectual property of ClaimRev and may not be copied or redistributed without permission.

7. Dispute Resolution

In the event of a dispute, the parties agree to mediation in Tulsa County before seeking legal remedy. Disputes may be submitted to small claims court if applicable.

8. Governing Law

This agreement is governed by the laws of the State of Oklahoma and applicable tribal jurisdiction, where relevant.
